Childfind Screening Applications are Available

  • Birth to age five preschool children not currently enrolled in the Grand Coulee Dam School District may be eligible for a free developmental screening.  Interested parents should request forms for a developmental screening and health inventory from Lake Roosevelt Elementary, Special Services Dept.  It is especially important for a parent to complete the forms if they think their child may be developmentally delayed, i.e. be in need of occupational/physical or speech therapy, or other possible health problems that may impede a child from being ready for learning.  Completed forms need to be returned to Lake Roosevelt Elementary, Special Services Dept..

Completed forms submitted by parents/guardians will be reviewed and a determination will be made as to whether the child needs to be scheduled for additional Childfind Screening and/or assessments.

Parents/guardians will be notified of results of the developmental screening as soon as possible.  Parents/guardians of children considered in need of additional services will be notified to schedule further assessments.

If your family is living in a temporary situation, you may contact the district where you are currently staying for a screening.

To request developmental screenings and health inventory forms please call Lyndsey Mitchell, 633-1442 ext. 306.
