Complaint of Harassment/Bullying Form
This form needs to be filled for any complaints of harassment or bullying by any adults such as parent to staff, staff to staff, staff to student etc.

Course Approval Form
This form needs to be filled out when teachers have clock hours or credits to turn in for placement on the salary scale or for certification purposes. Attach this form to the transcript and send to Amy Sayler.

Emergency Drill Form
Principals will use this form to report on the drills we have throughout the year. This report will help in making adjustments/improvements as we conduct the drills. Safety is of our utmost priority for our students and staff.

Employee Accident Form
If you get injured on the job and need to seek medical attention, please go see the doctor first and foremost. After you see the doctor, you will need to get an L&I packet from Amy Sayler. The form needs to be returned to me within 5 days of your injury.
If you get injured on the job but DO NOT need to seek medical attention, please fill out the in house incident report, have your supervisor fill out their part and return to Amy Sayler

Extra Pay Form
Please make sure to give your extra pay forms to your secretary so they can make sure to have the Principal/Supervisor sign off on them. Please do not send them directly to Debbie Van Geystel without a signature from your principal/supervisor.

Insurance Application
Fill this application out and return to Debbie Van Geystel if you plan to change medical plans, add or delete any family members or if you have any other qualified event happen. After you fill out the application, please be sure to go online to http://resources.hewitt.com/wea or call 1-855-668-5039 to finish the process.
